Der Streifen polarisiert: Die Filmzeitschrift „Schnitt“ bezeichnet ihn als „das heimliche Meisterwerk unter [seinen] Zwischenwerken“, Carsten Baumgardt von der Filmstarts.de- Redaktion meint im Gegensatz dazu, Shutter Island sei „zwar durchaus ein guter Psycho-Thriller, aber kein Meisterwerk mit Preispotential“. So gehen die Meinungen bei den Kritikern auseinander, doch dass der Film im Gedächtnis bleibt, ist unumstritten. Zu verwirrend, zu komplex, zu überraschend ist das Ende. Nach Vorführungen brechen im Internet Diskussionen aus, ob Leonardo Di Caprio in seiner Rolle als US-Marshal Edward „Teddy“ Daniels nun geisteskrank ist oder ob er schlicht resigniert.
